High Fidelity Wrap Around (HFW)

Through the wraparound process, a family and their team develop, implement, and fine-tune a plan of care that is individualized to achieve positive outcomes for the family. The HFW team consists of a care manager, family peer support and youth peer support. The HFW team helps the youth and family achieve the goals that they have identified and prioritized, with assistance from their natural supports and system providers.  The values associated with HFW require that the planning process, as well as the services and supports provided, should be individualized, family driven, youth guided, culturally competent, and community based.
