Care Management

Cayuga County is working with four health homes; Children’s Health Home of Upstate New York (CHHUNY), CNYHHN, Inc., Encompass Health Home and Greater Rochester Health Home Network (GRHHN). Health Home is a care management service model designed to assist individuals in their recovery journey, utilizing person centered practices and best practice health promotion services.  Care Managers refer, oversee and coordinate access to needed services, assuring that all needs are addressed in a comprehensive manner.  The length of treatment varies, but is determined by a child’s CANS-NY acuity and progress towards treatment goals.
